Indiana Dental Hygienist of the Year AwardFormerly known as Outstanding Indiana Dental Hygienist Award(Nominations due by March 1 of each year.)Award Criteria as of July 2006The IDHA Dental Hygienist of the Year Award is to be awarded “when applicable” to one dental hygienist in a given year. In a tie situation, the award will be given to both individuals. The resumes of selected nominees will be limited to the nominee’s professional activities with the greatest emphasis on association participation and may include a record of association services since initial membership. The nominator or the nominated candidate must submit no more than three pages, typewritten, covering the following: Component Activities, Community Activities, IDHA Activities, ADHA Activities, Special Activities, Employment Activities (Past 5 years).
